The forms below must be signed and returned to the AAFS. Your Student Academy contact is Nicole Zevotek (nzevotek@aafs.org). She will be reaching out shortly to provide you with a link to upload all forms. Any changes or questions to your registration should be communicated with Nicole. Schools will be invoiced for Student Academy prior to the Annual Scientific Conference.

Student Release
Each student attending must complete and sign the Student Release Form.
Anti-Bullying Agreement
Each student attending must complete and sign the Ant-Bullying Agreement.
Teacher/Chaperone Release
Each teacher/chaperone attending must complete and sign the Teacher Release Form.
