The forms below must be signed and returned to the AAFS. Your Student Academy contact is Nicole Zevotek (nzevotek@aafs.org). She will be reaching out shortly to provide you with a link to upload all forms. Any changes or questions to your registration should be communicated with Nicole. Schools will receive an invoice for Student Academy by the AAFS prior to the Annual Scientific Conference.
